Cutaneous plasmacytomas are clinically erythemato-violaceous cutaneous or subcutaneous plaques and/or nodules, with a smooth-surface, hard consistency, solitary or multiple. They are rare with less than 100 reported cases and can involve any area of the skin. We present a 64-year-old woman with a refractory IgG-Lambda multiple myeloma diagnosed one year before with renal, bone and central nervous system involvement. She developed rapidly erythemato-violaceous facial exophytic tumor lesions with necrotic areas at orbits, left malar and submental region with diagnosis of facial plasmacytomas. These lesions are often a reflection of high systemic tumor burden in the late stages of disease and are a poor prognosticator with most patients dying within 12 months (Figure 1).
