Parotid gland is the most common site of tumors of the salivary glands. The pleomorphic adenoma is the most common histological type of these tumors. The indolence of these tumors can be observed in large anatomical shapes in a low medical density observation. The authors report a case of exceptionally large tumor of the left parotid gland in a man of 53 years, evolving for 26 years. Tumor suggestive of pleomorphic adenoma has been a total conservative parotidectomy. the specimen had a weight of 1.500 kg , has actually proved to be, histologically , a pleomorphic adenoma comments. The tumor is a tumor called "giant". This is a rare clinical expression of the parotid tumor pathology. This observation adds to the few cases reported in the literature. The "record" seems held by Cachin Y, Institut Gustave Roussy (Villejuif ) for a tumor weighing 2,600 pounds. The indolence of these tumors associated with low medical density, explains the observation of these large tumors. Beyond the weight, the length of these tumors may induce a truly malignant carcinoma ex pleomorphic adenoma - determinism, which remains obscure. Whatever the size of a tumor of the parotid gland , the problems remain the same surgical technique . They are based on a correct understanding of the anatomy of the facial nerve, as recalled by Professor Malick DIOP (Senegal).
