80 years of old female presented with huge mass over occipital region for 10 years which was insidious on onset, gradually progressive in size. It was not associated with pain. There was no history of dizziness or numbness over upper extremities. On examination there was a huge globular mass measuring around 15 x 15 cm2 . It was soft, non-tender with smooth surface and no skin changes. It was not fixed to underlying structures. Fluctuation test and trans- illumination tests were negative. CT scan of the head and neck revealed large well defined cystic lesion with foci of calcification without enhancement on post contrast study suggestive of dermoid cyst. The diagnosis was confirmed with FNAC (Figures 1 and 2).
