A 23 year old male resident of Haryana is a known case of crohn’s disease for 2 years under follow up with gastroenterology department in our institution. He was on treatment with azathioprine since 2 years. He now presented with painful oral ulcers since 4 days duration. He was admitted and evaluated for disease exacerbation. His CRP was elevated but fecal calprotectin came out to be normal. UGI endoscopy was done which did not reveal any ulceration in esophagus, stomach or duodenum. He was started on 40 mg dose of oral prednisolone for 5 days. He improved symptomatically with disappearance of apthous ulcers; however, he developed oral candidiasis. He was treated for the same with oral fluconazole for 10 days.
