Patient is a 56 year old female with no known past medical history who presented to the ED with right eye blindness and pain. Upon initial workup she was found to have renal failure with nephrotic range proteinuria of more then eight grams. MRI of the eye was showed right Superior Ophthalmic Vein thrombosis. Later a renal biopsy revealed Stage 4 Lupus nephritis. What makes this case even more interesting is that this patient’s workup was unremarkable for anti-phospholipid syndrome.
